The THN Pharmacy Manager would be responsible for development of the Medication Reconciliation Process for the high risk patients involved in the THN network. Goal is to promote quality cost-effective outcomes for THN patients. Clinical pharmacist manager will provider medication reconciliation reviews on patients referred by THN Case Managers or THN providers. The Pharmacy Manager will check for discrepancies between the patient care summary and the patient medication list. Pharmacist will use EMR to review patient?s conditions, laboratory test results, medications at discharge and pharmacy claims data. The Pharmacy manager will communicate discrepancies found and resolutions to the discrepancy with the THN provider. The Clinical Pharmacy Manager will also be responsible for oversight of the quality metrics being measured within the THN contracts. Delivers excellent customer service utilizing Relationship Based Care to advocate for safety, patient preferences, and optimal communication through partnering with patients/families and colleagues, including all internal and external customers. |

Attends and/or presents at various local network meetings as requested such as THN Board of Trustees Committee meetings, Medical Management meetings and Director meetings. Serves as a clinical representative on Clinical Advisory groups. Provide oversite for the development and maintenance of Medication Management policies and procedures. -------------------------------------------------- Consistently documents all activities in the Electronic Medical Record using the established format in a timely and accurate manner. Promptly sends reports to physicians and other providers as per policy and as needed to relay pertinent finding. Maintains accurate accounting of work hours and miles driven to conduct THN business activities and submits in a timely manner. Actively participates in program improvement activities. -------------------------------------------------- Coordinates the education and training on staff in processes such as Medication Reconciliation and disease state management. Orient new employees to the THN's Pharmacy Program. Serves as a resource to network physicians and care managers on general drug information and Medicare pharmacy policy issues. -------------------------------------------------- Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens. Coordinates pharmacy activities in the care transition process to assure safe, effective, appropriate, and economical use of medications to improve continuity of care and outcomes. Coordinates pharmacy activities that support the Chronic Care initiatives and other future clinical initiatives. Coordinates clinical services provided to THN Partners. Oversees and coordinates the activities of contracted pharmacists within the local THN network. Serves as a resource to network physicians and care managers on general drug information and Medicare pharmacy policy issues. Oversees the THN pharmacy assistance program. -------------------------------------------------- Participates in Quality Assurance and Performance Improvement activities. Effectively applies the guidelines of the THN's performance improvement process. Directs the established process for assessing, managing, and reporting medication system issues.
