Overview

To provide clinical nutritional management to outpatient cardiac and pulmonary rehabilitation patients including nutritional assessment, patient counseling, review of laboratory values, and documentation of nutritional status. To provide nutritional consultation in other Heart and Vascular Center outreach areas. Nutritional services extend primarily to adult and geriatric populations.

Responsibilities


Perform nutritional assessment on all new Cardiac and Pulmonary Rehab participants utilizing the components of the Nutrition Care Process. Obtain nutrition and medical history and a nutrition survey from each patient. Measure and/or calculate height, weight, percent of body fat, BMI, waist to hip ratio and caloric requirements. Formulate an appropriate dietary plan within a month of patient's admission. Complete the Nutrition Focused Physical Exam when indicated

Initiate and provide patient education based on nutritional assessment. Recommend dietary changes and perform informal follow up with patients regarding dietary changes on a regular basis. Teach weekly diet classes for rehab participants. Provide additional community educational classes. Share nutritional information with departmental staff via formal and /or informal in-service education

Review laboratory blood values such as Lipid profile, glucose, A1c, as soon as accurate results can be obtained. Communicate results and dietary implications to patients

Document Nutritional Status Pre and Post nutrition behavior scores, Pre and Post laboratory values when possible and Pre morbid dietary habits and laboratory values when possible

Act as resource person and/or preceptor for employees, students, managing physicians and other staff as well as various health care teams

Qualifications


EDUCATION:
Required: Bachelor?s Degree in Nutrition or related field.

Preferred: Master?s degree in Nutrition or related field

EXPERIENCE:
1 year clinical experience, Required

1 year outpatient experience in related area, Preferred

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
REQUIRED

BLS (CPR)-American Red Cross or AHA Healthcare Provider

Drivers License | Valid Drivers License

Required:Certification as a Registered Dietitian/Nutritionist (RDN) as issued by the Commission on Dietetic Registration AND Licensure as a Dietitian/Nutritionist (LDN) as issued by the NCBDN. Certification as a Diabetes Educator (CDE) by the American Association Diabetes Educators (AADE) is required after three years of employment.
