Overview

 
Provides personal care services as a Nurse Aide in the Home Care setting. Assists with activities of daily living and home management tasks as outlined in the plan of care developed by the Supervising RN. Services may be rendered in the patients home or a facility. Reports directly to the Supervising RN. Talent Pool: Nursing Support

Responsibilities

 

 
Demonstrates personal accountability. Adheres to National Patient Safety Goals (including compliance with Infection Prevention, Hand Hygiene, Fall Reporting, etc.). Protects all confidential information and obeys all HIPAA regulations. Adheres to all organizational and departmental policies and procedures, including dress code and attendance. Documents care according to agency policy and submits documentation according to agency guidelines. Completes all departmentally required education, competencies, and/or proficiencies.

Performs direct patient care based on the plan of care while ensuring age appropriate care. Provides care for patients by providing assistance with activities of daily living: bathing, shaving, mouth care, skin care, toileting/incontinence care, repositioning to prevent breakdown for the bedbound patient, safety and monitoring with ambulation, progressive mobility and safe patient handling. Assists with incidental activities of daily living: light housekeeping, meal preparation, laundry, errands, etc. Keeps licensed staff informed of all responsibilities on an on-going bases, ensuring prompt notification in the event of a problem or potential problem or safety concern. Considers each patient?s physical limitations and developmental age providing special attention or special accommodations. Delivers care that is culturally sensitive. Adhere to agency policy for documentation.

Qualifications

 

EDUCATION:
Required -

High school graduate or equivalent

Completion of a state approved Nurse Aide 1 training

Preferred:

Home Care experience

 

EXPERIENCE:
Preferred -

One year experience as a nursing assistant

 

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
REQUIRED

Drivers License | Valid Drivers License

BLS (CPR)-American Red Cross or AHA Healthcare Provider

Required -

Current good standing with the NC Nurse Aide Registry verified through DFS
