Overview

Implements the Wound, Ostomy, Continence (WOC) Program by providing direct and consultative care for patients with wound, ostomy and continence problems. Collaborates with and provides direction to all disciplines involved with the care of wound, ostomy and continence patients. Assures program quality through direct intervention with wound, ostomy, and continence patients and through outcomes monitoring. Promotes cost effective care through application of knowledge and clinical expertise.

Responsibilities


Collaborates with the multidisciplinary team to plan and achieve clinical outcomes. Participates in QI for wound, ostomy, and continence clients to include data collection, outcome monitoring, trend reporting, analysis and action plan development.

Establishes a caring and therapeutic nurse/healthcare team/patient/significant other relationships. Recognizes communication barriers, identifies resources and establishes plans to resolve barriers to communication. Formulate individualized plans to address individual needs related to diversity, cultural difference or other unique variables. Advocates for the patient/significant other. Demonstrates skillful responses to customer needs based on particular context of the situation. Provides education to staff related to the patient/nurse relationship.

Reinforces and ensures appropriate patient education is carried out according to the plan of care. Assesses the educational needs for staff; plans and implements appropriate educational activities.

Utilizes the nursing process to provide direct and consultative care for acute and chronic wounds and fistulas, ostomies and incontinence.

Utilizes Wound, Ostomy, Continence (WOC) Program Protocols for pressure ulcer prevention and treatment. Recommends products and specialty beds/surfaces when needed as part of the treatment plan. Collaborates with the multidisciplinary team in product review and selection.

Qualifications


EDUCATION:
Required - Graduate of an approved RN Educational Program. Baccalaureate degree required in Nursing.
Preferred - Master?s in Nursing and required in 5 years from hire.

EXPERIENCE:
2-3 Years experience in staff nursing.
At least one year clinical experience specific to wound, ostomy and continence care.

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
REQUIRED
Drivers License | Valid Drivers License
BLS (CPR)-American Red Cross or AHA Healthcare Provider
Current valid North Carolina Registered Nurse License
Certification in wound, ostomy and continence specialty with ongoing certification maintained
