| Conducts comprehensive assessments of patients' needs to develop individualized care plans. Performs treatments following established protocols practices. Monitors and evaluates the progress of patients, documenting findings accurately and communicating any changes or concerns to the healthcare team. Educates patients, families, and caregivers on proper care techniques, prevention strategies, and signs of complications. Collaborates with physicians, registered nurses, and other healthcare professionals to coordinate care, ensure continuity, and optimize patient outcomes. Maintains accurate and up-to-date documentation of assessments, interventions, and outcomes in the electronic medical record system. Performs other duties as reasonably required or assigned. |
