The Supervisor Advanced Care Transitions oversees the day-to-day functions of the advanced care transition team to ensure that patients receive the correct care during their transition from one setting to another. This position is directly responsible for the supervision and coordination of daily care transition team activities, including: hiring, quality management, orientation, training, competency assessments, daily work assignments and campus-based outcomes. |
Talent Pool: Allied Health
Supervises advanced care transition team to check that patients are having their patient-specific and age-related recovery care needs met during transitions. Recognizes the need for front line staffing functions, flagging potential issues/conflicts to appropriate staff in order to prevent gaps in coverage before they arise. Provides guidance to all members of the Advanced Care Transitions team regarding daily issues and complex cases to increase the productivity of the team as whole, utilizing Advanced Care best practices. Proactively identifies areas where policies or procedures can be improved to increase efficiency, challenging the status quo for care if it is not effective and slows down the operations of the organization. Analyzes problems that arise in the department, searching for the root cause to eliminate the problem and help staff not have the problem come up again through training and guidance. Stays up to date with relevant rules and regulations to confirm that the Advanced Care Transition team stays in compliance with local, state and national regulations. Performs other duties as assigned. |
EDUCATION: |
Required: Bachelor's of Science in Nursing or Master's in Social Work |
EXPERIENCE: |
Required: 3-5 years of recent care management or social work experience |
LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
Required: RN Registered Nurse licensed in North Carolina OR Compact state OR Licensed Clinical Social Worker |
