The Clinical Pharmacist III - IV Ambulatory, works in a specialized ambulatory pharmacy area. They work with minimal supervision to provide patient care in their specialty area, engage with providers, nursing, and pharmacy staff and may supervise patient advocates and/or pharmacy technicians. |
*Outpatient cardiology clinic position *Clinical pharmacists provide direct patient care and independently manage chronic disease states including but not limited to: hypertension, hyperlipidemia, heart failure, anticoagulation, and weight loss *Assist with drug interactions and medication access *Precept pharmacy students and residents *Provide interdisciplinary department support for staff education and drug therapy monitoring |
Interprets and follows physician instructions to deliver the correct care to the correct patient. Provides and monitors drug utilization and medication-related information to healthcare professionals, patients and the public. Supervises and evaluates pharmacy technicians against the established goals of the department and greater hospital. Functions at a higher knowledge base and competency and spend the majority of time addressing complex situations or works on project improvement initiatives. Performs other duties as assigned. |
