Overview

This role is responsible for collaborating with the care team in identifying, assessing, and coordinating resources to address Social Drivers of Health and resource barriers.  VBCI (Value-Based Care Institute) Population Health BSW team members research and engage with local and state resources to determine availability and appropriateness of resources to meet patient care needs including but not limited to: food insecurity, housing, utilities, social connections, physical activity, and financial barriers. The BSW works closely with patients, families, and caregivers to educate and provide close and persistent support will connecting patients to needed resources to remove barriers impacting the patient's health and wellbeing. 

 

The Social Worker BSW provides social work services to help meet the needs of patients and their families. This role is responsible for the identification, assessment, and coordination of cases; and the anticipation of needs related to illness, hospitalization, finances, education, and home/community service.

 

Responsibilities

 

 
Advocates for patients to the appropriate authority or providers to get patients the highest quality of care that fits their needs and means.
Participates in quality improvement initiative to help raise the standard of care across their department and the organization as a whole, informing others within social work about best organizational practices.
Admits and discharges patients, checking that they have the correct documentation and know their next steps to best ease their path through and outside of the organization.
Manages the social work and care plan for various patients to collaboratively create a pathway for their treatment, both inside and outside of the organization.
Follows appropriate social work regulations, staying up to date with potential changes to allow for the organization to not fall behind on state, national or other forms of regulation related to social work.
Performs other duties as assigned.

Qualifications

 

EDUCATION:
Required: Bachelor's in Social Work

 

EXPERIENCE:
Preferred: 1 year long-term care experience
