The Congregational Nurse supports the broader community by addressing disparities in health and wellness in a clinic environment. Working under general supervision, this role promotes health and wellness for the congregations and community through access to physical and spiritual care. |
Assists community members in finding a primary care provider in order to connect them with access to care. Supports the health and wellness of members through personal health counseling to develop care plans. Identifies health disparities, including transportation, housing, and food to remedy issues that are preventing members from improving personal wellness. Educates members on disease prevention and management, medications, and the health system in order to increase knowledge and awareness. Organize health screenings and fairs and flu clinics to support the reduction of illness in the community. Completes screenings and assessments such as blood pressure and blood glucose monitoring to identify potential health problems for treatment. Provides spiritual care through prayer and listening in order to support a holistic lifestyle. Performs other duties as assigned. |
EDUCATION: |
Required: Bachelors Degree in Nursing |
EXPERIENCE: |
Required: 5 years of clinical experience in a health care setting |
LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider |
