The Birth Certificate Registrar serves as liaison between the patient and the county and state Vital Records offices. Working under moderate supervision, this position abstracts all information needed to complete the newborn infant's birth certificate, affidavit of parentage (if parents are unwed), and fetal death certificates meeting all state guidelines for completion, before the mother is discharged from the hospital. |
Assists parents by providing information concerning birth certificate procedures/state laws concerning unwed parents and their responsibilities and alternatives for completion of the Affidavit of Parentage (AOP), notarizes Affidavits of Parentage after their completion, faxes to state office and mail to the child support enforcement office. Delivers each Mother's Verification of Facts to the mother's room for review and signature, explains the proxy process, and offers parents to order their child's birth certificate. Handles all phone calls, questions, and walk-ins regarding the legalities of the birth certificate process. Maintains current knowledge on all state laws governing the birth certificate and fetal death process via attending annual Vital Records meetings, receiving Vital Record email updates, and by communication from peer birth registrars. Utilizes the Vital Records software to enter information for the birth certificate and state statistical data. Processes fetal death certificates through Vital Records website and obtains physician signature within state law timeframe. Performs other duties as assigned. |
