The Case Manager works with selected, sensitive populations to get them the highest quality of care that fits their needs. This position is responsible for: identification, assessment, and coordination of cases; and anticipation of needs related to illness, hospitalization, finances, education, and home/community service. This position develops a care plan that meets assessed needs in collaboration with the patient?s physician, other health professionals and the patient and family to ensure clients safety and care. |
Assists patients and refers them to resource and treatment pathways within the healthcare system to help progress their care. Monitors clients to confirm that they are receiving the highest possible quality of care, including collaborating with staff across the organization to check options for the patient's care, allowing all parties to make an informed decision. Completes and documents information about screenings, referrals and the care plan to keep records that are of use to both the patient and the system as a whole. Works with families and health professionals to develop a care plan that provides adequate care for the patient that ensures client safety and care. Refers patients and their families to community agencies for social supports, community health-based and other formal health programs (Medicare, Medicaid) to establish a path for future care for the patient, especially ones who are under socioeconomic stress. Performs other duties as assigned. |
EDUCATION: |
Required: Bachelor's in Social Work or Bachelor's of Human Services |
EXPERIENCE: |
Required: 2 Years of experience in the health or medical field related to home care |
