??The Program Manager ? Health Equity, in support of the overall health equity strategy of Cone Health, will provide program support and management oversight of health equity initiatives. This position combines strong project management skills with a working knowledge of health disparity mitigation strategies.? |
??Coordinates health disparity mitigation projects and programs, and community-based population health initiatives and strategic tactics with Cone Health to promote desired outcomes. Deploys community population health projects in targeted prototypes and develops strategies for spread and scalability. Supports the engagement of enterprise data analysts and quality improvement leaders; as well as engaging external community partners in developing population health initiatives and implementation of specific tactics. Pursues and helps drive meeting quality metrics. Responsible for the MDI board in the Center for Health Equity. This includes, but is not limited to, key performance metrics, drivers of success, and countermeasures. Provides education instruction and presentations for key communication initiatives within and outside the health system. Facilitates (agenda and planning) the program meetings, including Steering Committee meetings and sub-work group meetings. Serves as a liaison between designated internal and external stakeholders and the corporation. Leads and supports the program team by teaching, showcasing and supporting all projects. Works closely with the leadership to ensure all programs and updates are aligned for all customers. Performs other duties as assigned. |
EDUCATION: |
Required: Bachelor?s Degree in Nursing Preferred: Masters in Health Administration, MBA or Master?s in Health related field? |
EXPERIENCE: |
Required: 3+ years of responsibility for quality improvement initiatives and program management in healthcare, with proven leadership ability. Preferred: Lean management systems and A3 knowledge. 2 years project management experience.? |
LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
Required: Registered Nurse license in North Carolina or a Compact state Preferred: BLS (CPR)-American Red Cross or AHA Healthcare Provider |
