The Case Manager Registered Nurse (RN) provides comprehensive and coordinated care to patients. Working under moderate or general supervision, this role analyzes patient needs, participates in care coordination, conducts research, reconciles information, and reviews care plans. Responsibilities
|
Analyzes patient cases to assess their needs and to develop individualized care plans. Prepares and processes documentation related to patient care to ensure accuracy and compliance with regulatory requirements.
Researches and stays up-to-date with evidence-based practices and healthcare guidelines relevant to the assigned patient population to incorporate the latest knowledge into care plans and interventions.
Reviews and evaluates the effectiveness of care plans, monitoring patient outcomes and making adjustments as needed to optimize patient well-being to achieve desired goals.
Performs other duties as assigned.

Qualifications
EDUCATION: |
Required: Associate's Degree Preferred: Bachelor's Degree |
EXPERIENCE: |
Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations |
LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider |
