| The VBCI Population Health Care Guide Team Lead is a key member of the team, Care Guides provide support to the Population Health Care Management Team by serving as the communication liaison and patient advocate for patients/families, clinical care team and providers. They assist with scheduling activities and facilitating community resource connections to help ensure providers and staff are operating at the top of their license and capabilities. | 
| 1. Community Resources: The Care Guide Lead supports the care guide team in connecting patients to community-based and other resources that impact their overall health. Works with Health Advisors and other clinical staff to identify what community resources patients would benefit from. Connect with local resources to identify which community-based resources are available. Coordinates with the clinical care team. Communicates with the patient to ensure the patient has the information they need to connect with the community-based resources. Schedules necessary appointments or calls to connect patients to what they need. Supports patient in meeting the requirements of the community?based resource (i.e., fill out forms, collect documentation, etc.). 2. Supervisory Functions: Develops and amends monthly work schedule for care guide team members. Monitors and records staff time & attendance. Evaluates staff needs. Monitors reports for the Management and leadership team as needed. Participates in the functions of recruiting, hiring, performance improvement, and evaluating personnel with input from manager, physicians, and leadership as appropriate. Supports the manager in coordinating the training of all new care guides. Team Leader must be a super-user for our documentation platforms within the Population Health department. Supervision of all administrative personnel in accordance with overall health system and other regulatory requirements. Works closely with the Manager to ensure that operations are effective and consistent with the overall division. 3. Other Care Guide Functions: Patient forms, education, appointment reminders, and referral management. 4. Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional Affiliations. 5. Functions as a member of the interdisciplinary team, providing updates on trends and outcomes to the provider.  | 
| EDUCATION: | 
| Required: High School Diploma or equivalent | 
| EXPERIENCE: | 
| Required: 5-7 years of experience in a healthcare environment or equivalent university studies course work.  Preferred: Past experience leading a team.  |
