The Inpatient Care Management Department is a focal point of care delivery and is well-received and valued by the organization for our role in ensuring patients have a smooth transition of care back to the communities we serve.
The Care Management Assistant II is an exciting new role that will be utilized by the Inpatient Care Management Department to provide collaborative administrative support for clinical social workers and case managers by filling out appropriate documentation and contacting patients to ease burden of tasks off clinical staff and increase office-wide efficiency. The role will also have a key function for engaging our post-acute care community partners such as nursing homes and home health providers.

| The Care Management Assistant II provides support to Case Managers and Social Workers; responsible for informational accuracy in relation to patients and insurance, facilitate reimbursement and through-put of the patient stay. This role will also work to establish and maintain post-acute network relationships across the Cone Health enterprise. |
| Provides collaborative administrative support for social workers and Case Managers by filling out appropriate documentation and contacting patients to ease burden of tasks off clinical staff and increase office-wide efficiency. Prepares documents such as referral forms, using proper grammar and spelling to effectively communicate with other providers, both in and outside the organization. Corresponds with patients, caregivers, insurance companies, post-acute vendors and community agencies, under the leadership of a Case Manager or Clinical Social Worker (LSW). to ensure accuracy of information. Informs departmental leaders about potential gaps in coverage that social workers may have to note when there may be more resources needed and allowing departmental leadership to strategize on how to tackle said problem. Develops strong business relationships with Cone Health?s Post-acute network partners across the various care settings (SNF, LTAC, IRF, LTAC). Networks and contributes to business development with the post-acute partners, VBCI leadership, and Cone Acute facilities to provide a feedback loop on referrals and clinical and educational opportunities. Works in a collaborative manor with our Cone health acute care facility a cones post-acute resources, and our community partners to implement and measure an exceptional patient admission experience. Performs other duties as assigned. |
| EDUCATION: |
| Required: LPN training program or Bachelor of Social Work |
| EXPERIENCE: |
| Preferred: 1-2 years in Care Management or Post-acute support |
| LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
| Required: Current LPN licensure as required by state or BSW degree BLS |
