Access internal and external resources to meet patients and families discharge needs by maintaining a working knowledge of internal and external resources as well as financial, state and regulatory agency policies and requirements. Ensures timely access to all necessary resources for successful discharge. Discharge plans are communicated efficiently with the entire treatment team. Conducts regular discharge planning groups. Practices by professional code of conduct for Clinical Social Workers. -------------------------------------------------- Completes Initial psychosocial assessment and incorporates information into the plan of care. Independent in completing initial psychosocial assessment using all information available at the time. Collaborates with the patient to initiate the problem list and/or treatment plan. Updates treatment plan every six months. -------------------------------------------------- Conducts individual, couples and family therapy and completes treatment notes. Conducts sessions in a timely and optimally therapeutic manner. Formulates and communicates patients needs and prognosis to family members, when applicable. Practices by professional code of conduct for discipline. Ensures documentation is completed in medical record. -------------------------------------------------- Conducts utilization reviews with managed care organizations and documents in the medical record. 1-Conducts reviews within one visit of expiration to ensure authorization of a covered service including providing clinical updates to managed care organizations to obtain continued stay authorization according to standards. 2 Documents authorized length of stay to patient and other treatment team members as applicable. -------------------------------------------------- Coordinates patients care with referring physicians, community agencies and other therapists. Evaluates and updates patients progress in a timely manner. Effectively communicates patients progress to referral sources. Ensures physician collaboration in treatment process. Works closely with psychiatrists for medication management. Documents coordination of care in medical record. -------------------------------------------------- Develop and document treatment plan and updates. Collaborates with other disciplines in treatment planning process. Disseminates clinical findings to treatment team.
