Contracted population currently includes members of the following: Medicare Advantage Plans and Physician Provider Networks. Consistently documents all activities according to policy in the electronic medical record. Continuously demonstrates a positive attitude and understanding of disease management, supports team building, and is motivated to fulfill department objectives. Maintains ongoing communication of the care plan and timeline to the member and provider. Consistently demonstrates excellent communication and interpersonal skills. RN Health Coach will interact with the interdisciplinary team to include but not limited to: medical staff, patients, providers, families and caregivers. Assessments, health education and chronic disease management are performed by telephone at the THN Care Management office.
