Through the Attending Model, RNs can gain valuable leadership charge nurse experience through overseeing LPNs, while being supported by a 3/1 nurse ratio. LPNs gain valuable experience, can learn from RNs at the bedside, and typically pursue LPN to RN bridge program. NT’s benefit from better staffing ratios and a mobility specialist during the day. On 5 east, we highly value empowering our staff to go on to pursue career development through flexible scheduling for people in school, as well as a very generous tuition reimbursement program.   The 5 east team is a tight knit group of passionate and hard-working individuals. We have a shared governance team that meets once a month to discuss ways to improve the unit. We have a celebrations and decorations committee that likes to decorate for holidays and celebrate all that we do on our unit. We frequently send out rewards for staff members that go above and beyond. We have awards for staff to recognize each other, rewards for the top five people that spend the most amount of time in patient rooms, as well as for team members that are compliant with documentation. The ED Patient Intake Coordinator is responsible for facilitating the patient?s hospitalization from preadmission through discharge from the Emergency Department to the community or to an alternative level of care. The Coordinator interfaces with physicians, nurses, social workers (Care Management Department), and other healthcare team members to expedite medically appropriate cost-effective care. The Coordinator applies clinical expertise and medical appropriateness criteria to resource utilization, admissions and discharge planning. The Coordinator evaluates the medical necessity for admission, and appropriateness of patient care unit placement for all ED hospitalized admissions when on duty and provides consultation to physicians prior to patient placement. Upon discharge of the patient from the ED, will coordinate clinical and financial discharge planning needs as necessary so that a smooth transition from the acute outpatient care setting to the community setting is ensured and inappropriate readmission is averted. The Coordinator refers to Care Management Department patients with complex psychosocial or discharge planning needs and follows-up to ensure appropriate intervention. Dialogues with the payers as necessary to guarantee appropriate reimbursement. Communicates with physicians and other members of the interdisciplinary healthcare team to ensure appropriate utilization of resource and implementation of clinical practice guidelines or standing orders to meet expected standards of care for patients in the ED. Provides support and information to patients/families with identified financial or psychosocial needs. Serves as the liaison for the other members of the interdisciplinary team to ensure appropriate and expedient referrals upon decision to admit to the acute care inpatient setting. **Position may be non-exempt depending upon schedule (e.g., Weekend).

Under the supervision of the Director and the Coordinator, Clinical Pastoral Education, of the Pastoral Care and Counseling Department, the CPE Resident is able to function as a Chaplain, providing direct ministry to patients, their families and caregivers via spiritual assessment and support, pastoral and sacramental care. S/he visits each area of assigned clinical responsibility each scheduled day and responds to on-call requests following the schedule and procedure of the department. Has the demonstrated ability to actively and collegially function in relationships with peers and supervisor in the Clinical Pastoral Education (CPE) Resident Program. Approximately 75% of the student?s time is spent in direct contact with patients, their families, and staff. The THN Pharmacy Manager would be responsible for development of the Medication Reconciliation Process for the high risk patients involved in the THN network. Goal is to promote quality cost-effective outcomes for THN patients. Clinical pharmacist manager will provider medication reconciliation reviews on patients referred by THN Case Managers or THN providers.
