The THN Pharmacy Manager would be responsible for development of the Medication Reconciliation Process for the high risk patients involved in the THN network. Goal is to promote quality cost-effective outcomes for THN patients. Clinical pharmacist manager will provider medication reconciliation reviews on patients referred by THN Case Managers or THN providers. The Pharmacy Manager will check for discrepancies between the patient care summary and the patient medication list. Pharmacist will use EMR to review patient?s conditions, laboratory test results, medications at discharge and pharmacy claims data. The Pharmacy manager will communicate discrepancies found and resolutions to the discrepancy with the THN provider. The Clinical Pharmacy Manager will also be responsible for oversight of the quality metrics being measured within the THN contracts. Delivers excellent customer service utilizing Relationship Based Care to advocate for safety, patient preferences, and optimal communication through partnering with patients/families and colleagues, including all internal and external customers.
