Program Management

• Is knowledgeable of latest stroke certification requirements and guides stroke certification initiatives for each Cone Health site to ensure programs remain compliant
• In collaboration with Stroke Medical Director, oversee appropriate data entry, abstraction, & data integrity
• Monitors continuum of care for stroke patients to encourage stroke prevention & appropriate access to stroke follow-up
• Facilitates multi-disciplinary stroke committees
• Establishes relationships & provides updates leaders from applicable EMS agencies, EDs, inpatient nursing units, & other applicable departments.
Performance Improvement
• In collaboration with Stroke Medical Director, leads creation of annual performance improvement plan and ensures it is updated regularly.
• Proactively identifies performance improvement opportunities & facilitate performance improvement initiatives
• In collaboration with Stroke Medical Director leads development, implementation, monitoring and revision of policies, protocols, care plans, and order sets for stroke patient care
• Organizes, analyzes & reports stroke outcomes to relevant committees
• Supervises & facilitates regular case reviews
• Participates weekly in leader rounding on stroke patients to assess stroke patient’s satisfaction & appropriateness of care
Support & Supervise Staff
• Provides guidance & support to Stroke Program staff to ensure they are informed & able to effectively perform their responsibilities
• Ensures staff meet HR requirements & adhere to corporate policies & values
• Address & resolve needs/concerns of staff in timely manner
Education & Clinical Resource Activities
• Leads planning of annual stroke conferences
• Coordinates educational activities & campaigns for patients, families, staff, physicians and community.
