Maintains ongoing communication of the care plan and timeline to the member and provider. Consistently demonstrates excellent communication and interpersonal skills. RN Health Coach will interact with the interdisciplinary team to include but not limited to: medical staff, patients, providers, families and caregivers. Assessments, health education and chronic disease management are performed by telephone at the THN Care Management office.
